Frost line 4 1-2’ deep. Residential house 40’x60’. Post frame concrete piers. 4” floating slab with radiant pex inside it. On top it f 2” rigid in top of 10mil poly on 6” crushed rock granular A where plumbing pipes run. 2” rigid insulation also running 2’ vertically at the perimeter grade board. Must the pipes for plumbing be specially insulated around? And must the main exiting 4” leave the house below frost line?

I assume you'll be running glycol in those lines, if so, your pipes won't freeze. As for the 4" main, normally we keep those as close to the surface as possible, but even if it goes below frost line, I don't think it matters all much
